first of all I have to see preop photos .You may have a cicatricial alopecia , so do you have a skin biopsy before ? The most outstanding reason for poor growth and ineffectual hair restoration results is insufficient number of viable graft placement as a result of intra operative damage of follicular units ( during insertion or high transection rates during harvesting, long time of grafts out of body, dehydratation, poor FU storage/handling conditions like high room temperature, storage fluids etc.)

The transplanted hair should not fall out unless there are other medical issues casing this. Usually the deterioration of hair is from continued loss of native hair. This loss of native hair can be slowed down by using minoxidil and finasteride either topically or orally.
