Platysmaplasty is the suturing of the neck "band" muscles together. This is done with a small incision beneath the chin that hides beautifully. The result is typically lifting of the neck area to a modest extent, depending on how much skin excess there is. With lots of skin, particularly sun-damaged skin that is thin and has lines, it is difficult to get much of a result without some type of facelifting. Submental liposuction (removal of fat through a small instrument called a cannula, via a very small 3mm incision) can be done alone or together with platysmaplasty, depending on your individual anatomy. If you only have fullness of a little bit of excess adipose tissue in the neck, and you have youthful skin, liposuction alone can provide a beautiful result. A platysmaplasty is the tightening of the platysma muscle.The platysma muscle is what causes the banding in your neck as you age (turkey wattle). There are layers of fat that lie over and under the muscle, so during a a neck lift, the platysma muscle is tightened (platysmaplasty) from an incision under the chin (in a natural crease), and liposuction is performed in the same area to remove the fat. This combination of procedures is excellent to improve the neck contour and profile. It can be performed in conjunction with a facelift or on its own.
Submental liposuction/liposculpting is performed by making an incision under the chin (in the natural crease) and removing the fat between the skin and underlying muscle (platysma). This fat removal can be performed either by a liposuction cannula (straw-like tube), by direct excision of fat, or by a combination of both. Although rare, a surgeon may remove additional fat deep to the platysma muscle (subplatysmal fat) if he/she feels it is necessary to obtain better results.A platysmaplasty, by definition, is anything that is surgically done to the platysma muscle that alters its anatomy. Most commonly, prominent platysmal bands (ie cords) and sewn together to remove the bands and increase the angle and definition under ones chin. Other times the platysma muscle (or bands) can be cut/injured so that they are not as strong/prominent. When this is performed under the chin, this is technically called a "medial" platysmaplasty. Any manipulation of the platysma from the sides (ie around the ears) would be considered a "lateral" platysmaplasty.

Sub Mental Liposuction- It is a surgical procedure where excess fat in the neck is remove by negative pressure suction attached to cannulas with holes. It will define the neck and jawline by removing excess fat and causing the skin to tighten overtime.

Platysmaplasty (with or without some liposuction in the neck) is one of the best techniques to dramatically improve a sagging neck, get rid of vertical bands, and improve the neck angle when viewed from the side.The platysma muscle is shaped like a broad flat sheet. There is one on each side of the neck, and they begin over the collar bone then extend upward just over the bottom edge of the jaw and actually then blend in with some of the deeper support layers in the cheek.When we are younger the two middle edges of each platysma muscle are closely connected in the midline just under the chin. With age the middle edges of each muscle begin to drift apart. This often leads to the vertical bands that people notice in this area. The weakening of the platysma leads to a loss of support in the neck causing sagging which is made worse by excess loose skin. Platysmaplasty is a type of necklift technique to tighten the platysma muscles and provide support in the neck to restore a youthful contour. Many techniques have been tried over the past few decades, but them most reliable methods involve using sutures to bring the middle edges of the platysma back together. Once that is done you may also need the side edges of the platysma tightened as well.Any work on the platysma in the midline is done through a small incision hidden just below and behind the chin. This incision can also be used to perform some liposuction if there is any extra fat in the neck that needs to be removed.One thing to remember:A platysmaplasty is a great way to tighten the deeper support layers in the neck, but it does nothing to address loose sagging skin. If you have that going on then the extra skin may also need to be removed as well. Your plastic surgeon should be able to guise you through what is best in your specific case.
A platysmaplasty and submental lipo is performed in patients that have fullness and loose muscles of the neck. Typically patients may be in their late 30-40's. Older patients will require a platysmaplasty and submental lipo but ALSO will need skin excision in the form of facelifting and neck lifting. Platysmaplasty is suturing together the loose muscle bands that are commonly called the "turkey neck" bands. Submental liposuction is suctioning the skin fat from underneath the chin which is the "submenta"l area.
Sumentoplasty or plastymaplasty is the direct treatment of the neck where fat is removed by both liposuction and direct excision. Then the platsymal muscle is sewn together as the final step to create a tighter and more uplifted neck angle.
Liposuction is the removal of fat, and "submental" refers to the neck and under-chin area. Fatty deposits in the neck may become excessive just underneath the skin and above muscle, or underneath muscle; these deposits can lead to the appearance of a double-chin. Platysmaplasty is the surgical sewing and tightening of the neck muscles called platysma. All patients have 2 thin, broad, sheet-like platysma muscle which stretch from the lower border of the jaw to the collar bone; these 2 platysma muscles are bound together in the middle of the neck in the young patient, and tend to separate in the older patient, leading to vertical neck lines. Correcting these lines involves a platysmaplasty, which usually involves placing a corset-like suture to the platysmal muscle edges underneath the skin. Submental liposuction can be performed alone in the young patient with an isolated under-chin fatty deposit and taut skin; the same procedure is often combined with a platysmaplasty in a neck lift, where muscles are tightened and excess skin is removed.
