The best procedure will likely be a revision reduction procedure, your previous scars can be used and the goal would be to help lift the breast tissue that has started to descend, "bottom out" to a higher position, and also remove some of it. If you are happy with the size of your current breasts in a bra then you may just need a mastopexy/lift without removing additional tissue, which again likely can be done using your current incisions.

Based on your pictures it look like you are healing well, just give it more time for them to settle and continue to follow up with your PS to discuss your concerns.
This is not unusual and with regular wound care and dressings this will heal. you need to keep in touch with your PS but generally one would not debride at this stage.
At this point in time, your scars should have already healed and you may already expose it. However, a sunscreen would be recommended for the first year to avoid hyperpigmentation that could darken your skin when exposed to sunlight.
