I had a breast reduction 4 months ago, going from a 34F to 34C. I am thrilled with them when I am wearing clothes and noticeable difference in weight, however I am very unhappy with the shape of them as they are lumpy at the sides and base. I had the dog ear removal a month ago but it has not really improved the shape. My surgeon seems happy with the results and does not need to see me again I don't know whether I am being unrealistic as it was a big reduction. Any advice?

You are probably too early post breast reduction to decipher the final shape/appearance. I would usually wait at least 6 months before contemplating any revision surgery. It's still early days, 4 months postoperatively your breasts have not reached the final shape, it will take longer to get more stable and closer to the final outcome. Also, the fact that you describe lumps shows that the scar tissue is still active and you might also have a degree of fat necrosis. I think you need to voice your concerns with your surgeon, you might need some scans to follow up the "lumps" and you might also need to wait a bit longer for your final result. Scar and wound healing takes a year after any surgery, and so you are still in the relatively early stages of healing at 4 months postop. If at the 1 year mark you are still bothered by the appearance re-address your concerns with your surgeon, or seek a second opinion to see whether you'd benefit from some sort of revision (which can include a combination of liposuction, scar revision, and/or formal surgical excision of excess tissue). If you do decide to go to a different surgeon be sure to take a copy of your operative report with you, so that your new surgeon can tell what "pedicle" your initial reduction was based on (which helpd with planning to keep the nipple/areola alive).
