In every professional picture taken of me, I notice how saggy my cheeks and mouth are. I also despise how square my face is. What surgeries do I need to correct these? I am 18 but I still haven’t “grown into my baby fat” so I am losing hope. My face is so saggy that I have a resting sad face that always gets comments.
Answer: Timing for surgery Thank you for your question and pictures. Just because you have turned 18, doesn't mean your facial growth has completed. People reach facial growth maturity at different times. I would highly recommend for you to NOT rush into surgery unless your facial features have been stable for at least 1 year (preferably 2 years). If you are sure that you want something done, then I would start with treatments that are not permanent such as fillers. You will be best served by seeing a board certified plastic or facial surgeon to discuss your concerns and possible treatment options.

Answer: Lip lift You are quite young at 18. One thing you can consider is a lip lift. A lip lift does 4 things: shortens the distance between your nose and upper lip, increases the mucosa of the upper lip, increases the incisor show and pulls up the corners. Best Wishes, Gary Horndeski, M.D.

Answer: 18-year-olds needing facial surgical procedures Facial aesthetics are primarily determined by bone structure, rather than by soft tissue coverage. Each person will have their own unique facial appearance, and this is primarily determined by their skull. Changing your inherent facial appearance is quite an involved endeavor, and generally only done for those who have significant Deformity. Avoid looking for simplistic solutions to complex problems. Based on your pictures I don’t see a need for surgical intervention but rather reassurance. Best, Mats Hagstrom, MD
