18, female, 5’6, 120 lbs. An example of the kind of button nose I mean is Kristine Froseth’s — is it possible for my big nose to become that small? I’ve heard about bulbous noses with thick skin being impossible to really fix, although I don’t know if my skin is thick, and I never see photos of my kind of nose in before and afters. I feel a little hopeless and I just want to know if I can actually fix this.

Answer: It should be possible to narrow your tip. More ... See the before and after photo that I posted with this response. The tip of the nose is made up of two cartilages, and yours appear to be strong and broad, and there are indeed ways to narrow the tip of the nose. You only posted a couple photos, but often, when the tip cartilages are strong, besides making the tip appear wide, they also can hold the tip down, so elevating the tip is sometimes a good idea, and they can hold the tip out away from the face, so bringing the tip back closer to the face can be good, too. How *much* to narrow or elevate the tip, or deproject the nose, or make other changes? We figure that out with morphs, well before surgery. I'd love for you to see some excellent professionally-designed morphs of what could possibly be done with your nose. Morphs could also help you identify better just what's bothering you, and help you set a goal for the rhinoplasty that's accurate for your tastes. Profile and three-quarter views would be particularly important in morphing your nose. (Side note: in my opinion, morphs should really be done by the surgeon, or he should direct an assistant as she makes the morphs. Morphs should be made with a constant eye to what actually *can* be done in surgery, for that particular nose, and the surgeon has that information and judgment best.) Finally, remember that rhinoplasty is an exquisitely difficult operation to get right, and you should only have surgery if you are able to make yourself very confident in your surgeon's skills. The changes that your nose needs require advanced techniques, and skill that most plastic surgeons don't possess with expertise.
