Be very careful. You may see well meaning surgeons who propose to repair this with lateral canthoplasty or lateral canthoplasty with a spacer graft. In my opinion, you need more resources to correct this issue. These approaches will be insufficient here. You are short of skin and soft tissue for the lower eyelid. Some surgeons will propose a skin graft to lengthen the lower eyelid. Unfortunately a skin graft is very unsightly. To get the most aesthetic repair of this post blepharoplasty lower eyelid retraction, in my opinion, you need to permanently recruit skin and soft tissue into the lower eyelid, the weight of the cheek needs to be removed from the lower eyelid. Finally, the lower eyelid needs to be shaped and the lateral canthus repositioned under no tension. This shaping is performed with a hard palate graft that is sewn in behind the lower eyelids. I am attaching a video of my surgery that I developed that reliably repairs these eyelids and a reference to my website.
