Without seeing your photos I cannot advise you personally, but a combination of fillers, botulinum toxin, skin peels and Ultherapy +/- CO2 Fractional Laser Resurfacing will give best outcomes. Alternatively consider a surgical approach.

Icing for several minutes throughout waking hours day of and day after the procedure feels good and wont affect the outcome and tightening should start to become apparent eight weeks post treatment.
Ultherapy can be variably uncomfortable depending on your sensitivity but is no where close to laser treatment. I think injections of Demerol or Toradal prior to the procedure are a bit of overkill but don't really object to it. I really don't want my patients to be uncomfortable so...
