Question

2nd Revision - would like spreader grafts and perichondrium removed. nose looks crooked and wonky! i am 12 months postop. I don't like the result because my nose is bulkier, wider, crooked and has a wonky appearance. I want to know if spreader grafts can be removes safely and if so, will the bridge go back to how it was prior (please note that i did not have my bones broken or bridge rasped previously). I was able to breath find before as well. Also, i'd like to have the perich. removed from my supra tip break and tip. This has made my nose bigger and has taken away my contours. Is this possible with the right surgeon?

The feasibility of tertiary revision rhinoplasty for a narrower nasal appearance may require a consultation. Consider meeting with several rhinoplasty specialists, and bring a copy of your previous operative reports, and preoperative photos. Hope this helps. Dr Joseph

To answer your question, yes spreader grafts can be removed, partially and completely. Based on the before photo, it appears like you have very thin skin. In the first photo the upper 2/3 is too thin for the lower 1/3 of the nose. In the second photo they are more in harmony, but your nose is a little wide for your face and slightly asymmetric. To address your question about the supratip/tip, I think that area would look best somewhere in-between its current state and the before picture, so you can have some definition without the sharpness. But that is just my personal preference. Given that this will be your 2nd revision, I would recommend consulting with someone that specializes in rhinoplasty (especially revision rhinoplasty) so that a full consultation/examination can be performed. Overall, you want a slimmer, more defined nose-- determining the status of your anatomy is the best way for a surgeon to help set realistic expectations. Best of luck on your journey!

Your nose is wide and asymmetric. While the spreader grafts can be completely remove my suggestion is partial removal and more on the left than right to correct the asymmetry. Your nose was too narrow before the surgery.

While the spreader grafts certainly can be removed, the original nose was too narrow, therefore a partial removal may be helpful. Also important to address the asymmetry in the mid-vault of the nose as well

Your nose is presently too wide. Grafts can be removed totally or partially to improve the symmetry .A common question I get is can we go back to the same point as before the surgery. The appropriate perspective  is your present state is the starting point and most importantly  there are several things that can be done to improve the situation.
