I feel like they are wide set, or possibly "east west" breasts. I feel like if they were more centered they'd look more normal and full. I'm a 32C cup, but if feels like the sides of my breasts could be transported to the middle haha. Is there any way to rearrange tissue? I don't really want unnatural implants unless necessary. What would I do?

In my 28 years of experience as a board certified Female Plastic Surgeon in Chicago and the Northshore of Chicago, I have helped thousands of women understand their options for creating more centralized natural appearing breasts. It is always best to have an in-depth consultation so that your Plastic surgeon can best explain your options once you have been examined. The key is for your Plastic surgeon to understand the relationship between the position of your breast tissue, your nipple size, shape and position on your breast and how your breasts and nipple are positioned on your chest wall. Your unique anatomy and your expectations need to be aligned for your Plastic surgeon to then offer you options. Often times a low profile implant of adequate width and small volume (size) can be positioned to provide more lateral breast appearance, thereby centralizing your breasts and nipples, while providing natural cleavage. The wonderful thing is that there are many implants that are wide enough and have low volume that meet this criteria and will provide you with a natural looking breast. The key is that your Plastic surgeon understands what you want and has the experience to achieve this outcome. I have web referenced a case in which I used a 190cc low profile implant in a women who had widely positioned breasts and nipples and merely wanted her breasts slightly more full and more centralized. Because I created a lateral pocket in addition to a medial pocket, the implant which had a wide width functioned to create lateral breasts, allowing her nipples to look more centralized. Answer: Cleavage Dear bailey0098,Many women seek improved cleavage. Narrowing the cleavage plane can be done very carefully by a skilled surgeon. Moving the implants too far medially can result in synmastia or unaboob which is very difficult to correct. That being said, the right amount of pocket dissection and implant selection can create excellent cleavage. In order to create a more centralized appearance of your breasts, an implant could be placed to achieve this. The size, profile, and shape of the implant is based on your desired breast size/shape, your chest wall measurements, and soft tissue quality. This decision should be based on a detailed discussion with equal input from both you and your surgeon. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and evaluate their results. The most important aspect is to find a surgeon you are comfortable with. I recommend that you seek consultation with a qualified board-certified plastic surgeon who can evaluate you in person. Answer: Thanks for your question. Other than augmentation, the only other procedure that might possibly achieve the appearance of more cleavage and more central fullness is fat grafting. This means borrowing some extra fat from wherever you feel you can spare it (tummy, thighs, buttocks) and transferring that fat to the desired areas. This may require more than one procedure, and is generally less effective than breast augmentation. That said, as indicated by several of my colleagues, even though you may not be happy with your breasts, take comfort in knowing that they are entirely "normal". One word of caution. As you consider augmentation, based on your photos alone I would recommend a base width sufficient to provide adequate medial fullness, without making you significantly larger. This would probably mean selecting a moderate or low profile implant. Furthermore, there is a slight danger that with more medial fullness an optical illusion will be created that makes your nipples appear even more laterally position than they already are. Make sure you select an experienced board eligible or board certified plastic surgeon whose results appeal to you on his or her before and after gallery. Do not be reluctant to seek more than one consultation to make sure you are confident with your treatment plan and with your surgeon. Best of luck to you.
