Fat transfer or Sculptra in sunken lower cheek (next to mouth)? (photo)

Did you have a facelift? That can often create the "deformity" you are seeing. In any case, fat transfer is far superior to correcting this than Sculptra. And fat is permanent.

Thank you for your question and photo. Fat transfer might be your best option for this area. This procedure starts with the removal of fat from one area of the body via liposuction. Common donor areas are the abdomen, hips and thighs. The fat is then processed and prepared for injection into the face. The advantage to fat is that it can last much longer than any dermal filler. However, the results can also be less predictable. Make sure you choose a very experienced, board-certified facial plastic surgeon for your treatment to ensure an optimal outcome.

I appreciate your concern, and physical exam would be optimal, but you have 2 main options. Fillers like Juvederm and Voluma work well but are temporary, and fat works well, lasts mostly forever, but requires a procedure. In your case it appears fat transfer would be the best option.. If you have enough fat that is the best choice: longest lasting and least chance of undesirable side effect but both are good choices.   Part of your problem is skin laxity which will not be corrected of course.   Your area of concern can be  effectively treated.   Fat transfer is an office procedure done under local anesthesia, sometimes with oral sedatives as well.

Fat would likely be a better choice in that area. You said that you did not want temporary filler but sculptra it is a temporary filler. Fat will have a good chance of lasting a very long time. Fat transfer to fill an area of volume deficit is an excellent choice when you have a source of fat elsewhere. Sculptura is nowhere as good unless you have no source, the reason Sculptra was developed. You appear to have laxity of the tissues also. If so, you may also need a procedure to tighten the area, but only an in person exam will determine this. Make sure you see a board certified plastic surgeon with significant experience in surgical and fat grafting facial rejuvenation.
