Cheeks overstuffed after fat grafting?

Removing fat from a grafted area in the face can be extremely difficult especially if there is no obvious lump of fat that is palpable and or visible, which could be removed either via liposuction or by direct excision. Most of the time, the grafted fat is closely associated with the muscles and  likely impossible to remove with precision. Performing fat dissolving injections in the area of over treatment may perhaps reduce some of the excess volume since the target molecule of the active ingredient is fat, however, the resulting contour would be difficult to predict. There is a certain radius of action of fat dissolving products such as Kybella and, theoretically, one can postulate that as long as the injected product is well beyond that radius of activity the perhaps the buccal and jaw areas might not be affected, but there is no guarantee.

 Removing grafted fat in the face can be tricky and at times not possible.  You might find someone willing to inject Kybella as an off label use of the medication but I didn't know of no one what does this procedure. At times liposuction may be an option but this mostly depends on if the area has natural fat that can be removed using this technique since grafted fat is often in bedded in muscle and other tissues. My recommendation is generally for patients to stay away from facial fat transfer unless they have chosen to provide her with great care.  I see far too many posts about patients wanting to have a fat removed from their face after this procedure to think that it delivers consistent results except for in the hands of those few who are true expert's.  Best, Mats Hagstrom M.D.
