Best surgery for superior jowl fat? (photos)

Question

I have had my lower jowls treated and they are no longer an issue. Unfortunately, I still have bulging beside my mouth in the superior jowl fat region. Considering I do not want the lower jowl or jawline treated, is there a surgery that would be best just for treating this area? I think that fat used to sit further back in a slightly diagonal direction.

Thankyou for providing reference pictures for a better recommendation. It seems as that you would benefit from a mini facelift which would target your midface and lower cheek area. This procedure will give you a more defined jaw structure and pull your jowls to get a tightened and firm look which would look more natural. I would also suggestto look carefully into your surgeon of choice and make sure they are doing scarless vertical endoscopic lift which would be give more natural looking appearance.

Yes sometimes that can be treated with a bit of lipo to the area to reduce the fullness, some clinics offer microneedling and botox to reduce the appearance of this. More photos would be needed such as the other side of the mouth and side angles. Speak to your plastic surgeon about some options that they may offer.

Thanks for sharing your photos. It is extremely illustrating of the concern and remedy. As we age, particularly after 30, restoration of collagen and other structural skin molecules diminish. The mobile skin and underlying soft-tissues are teathered to the deeper tissues centrally and along branches: forehead and lateral eyes, nasojugal fold, nasolabial fold, marionette fold and central neck.  With gravity and passive skin laxity, particularly with weight loss and / or lipo volume loss, the skin sags into folds. The “superior  jowl” fat you speak of is that fold of looser skin and fat between the nasolabial and lateral commissure/marionette folds.

Options include:
Lifting as you show in the second photo. Consider ThreadLift, Cheek volume filler; facelift with need for excess skin removal
Overall tightening of the face: retinoids, chemical peels, Sculptra
