Facelift with upper / lower bleph? Or fat transfer?

Question

I am currently scheduled to have a facelift and am feeling very nervous about it. The doc originally said he thought mini facelift and fat grafting to temples, cheeks, tear troughs, and nasolabal folds. Another doctor recommended upper & lower bleph and mid facelift, which seems more on point given my face. I feel like my cheeks need lifting, and I don't like the festoons on my upper cheek. The 2nd doc says it's filler, but when that's removed, won't it be more wrinkled?

There is no full face photo so I can't comment on the lift.  However, with adequate volume replacement, you may not need the lower bleph.

People with fat transfer that ends up visibly lumpy. I’ve done over a thousand facelifts and never fat grafting. I’ve yet to have a patient complain that their face is too thin. Fat transfer is a waste of money. If the surgeon insists then just go elsewhere

Much more information is needed, such as a full set of facial photographs from all angles including your neck. In our practice, the goal of a lower face and neck lift is  to tighten loose facial and neck skin, tighten loose facial and neck muscles, lift the jowls, and remove fat deposits in the neck located above and below the platysma muscle in addition to a platysma -plasty. Mini facelifts give mini results, and we rarely ever perform them. For a  variety of reasons we also don't perform fat injections and mid – facelift due to complications that can occur with these procedures.
