One year post revision for double bubble - Does this look like CC? (Photos)

Question

Had my BA in November 2024. Then went in for a revision to correct double bubble in April 2025. My PS did internal bra sutures & the Right breast is fine— But we call the Left SpongeBob Square Boob. It looks and feels like the sutures have failed on the outside “corners” of the implant. When he did the revision, he noted how surprised he was at how thin my skin was. If I don’t wear a heavy duty bra or go braless for an extended period of time, it feels like the implant is going to pop through the skin and is painful. There are visible and palpable ripples in those “corners”— that’s where it becomes painful. The inside cleavage of the left is also very flat-— like a flat wall coming off of my chest, it does not have the same curve that the right does. Does this look like it could be Capsular Contracture? I haven’t noticed any firmness, but looking at them it seems like it could possibly be an early grade.

This doesn’t sound like classic capsular contracture, since you’re not describing significant firmness or tightness. What you’re noticing&mdash;rippling at the edges, a flat inner cleavage, discomfort without support, and the feeling of the implant pressing against thin skin&mdash;fits more with thin tissue coverage and possible loss of internal support or pocket control after your revision. The comment about your skin being very thin is important here. I’d strongly recommend an in-person review with your surgeon to assess implant position and support, as this may require reinforcement (like internal sutures or mesh) or improving soft tissue coverage (such as fat grafting), depending on what they find.

I understand your concern. However, without a proper assessment, it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

To diagnose an early capsular contracture requires a physical examination. As you describe your condition this sounds unlikely. The thin tissues you describe may be the source of your problems and the options are limited. A return visit to your surgeon would be appropriate at this time to discuss possible options.

I encourage you to return to your surgeon for an evaluation.  Your description doesn't sound as much like capsular contracture but more like issues related to the thin skin in the area that concerns you.  Capsular contracture can make your breasts feel firm, change shape, and even hurt.  Your surgeon will advise what is recommended after an exam.
