Provided you have enough buttock projection, then liposuction to sculpt the upper buttock love-handle waistline area and liposuction to the lower outer buttock would be your best place to start. Encourage my patients to do this so I know what they want to achieve. You are absolutely correct with pic # 3 being the outcome. You could have thigh liposuction to get you half way between picture 3 & 4. If you lost weight or got liposuction the point to achieve # 4, you’d more than likely end up needing the extra skin removed at the fold where your buttocks meets the thigh and possibly skin off your abdomen, not recommended as #3. From the simulation photos of your goals you provided, you would need to address your back, buttocks, hips, and inner thighs to get this result. Although it is difficult to say with complete certainty since I have not examined you myself to asses for any skin laxity, I would recommend liposuction of the upper and lower back/waistline, inner thighs, and lower buttocks along with fat transfer to your hips to give you that rounded hip contour. I would also recommend RF skin tightening to the lower buttocks in order to address any skin laxity and help shape the lower buttocks. Make sure to consult with a board certified plastic surgeon and good luck!

It does not look reasonable to expect your ideal shape with your current weight; this may not be pleasant to hear, but that is your reality! No liposuction can achieve such a perfect result as 15-20 lbs. weight loss! Once satisfactory weight is achieved, a combination of liposuction and, possibly, fat grafting, will help to come close to your goal. This change is created with an excision of skin called a tuck.
It may be challenging to find plastic surgeons who have a lot of experience. The procedure is well described by Dr. mendieta on YouTube video called the Miami thong lift.
Otherwise the process of finding the right surgeon means contacting a lot of providers who do the BBL procedure or gluteal enhancements and ask how much experience they have doing this. during in person consultations ask providers to show their entire collection of before and after pictures of previous cases including long-term outcome.
It may be challenging but I’m sure there are plenty of people who do this procedure. dr. Stanton in Southern California who is a big implant guy talks about it sometimes in his posts.
