Hello, I had a reduction in 2016 I came out of surgery still a DD from a 36H Along with my reduction I had side rib/arm lipo for the side boob. I found zero results from the lipo I hate how saggy the breasts have gotten and how large they are around the base Can I get a second reduction to almost nothing then smaller implants placed? I much prefer the look of implants over real as mine are so saggy all around
Answer: Breast reduction then implants Thanks for your question! That is always an option for patients that want to be more breast implant than breast tissue. I would suggest seeking an in-person evaluation to determine if that is the right course of action for you. Good luck!

Answer: Considering options I encourage you to begin by visiting with a few board certified plastic surgeons for consultations. There are multiple options to consider. It is possible to have a reduction and also have breast augmentation. Depending on your goals you may be a candidate for autoaugmentation. This is a breast lift (with or without reduction) with a twist. It uses your own tissue to give upper pole fullness. This is not a fat transfer. I've included a video on this approach that I hope you find helpful.

Answer: Redo surgery to get upper pole fullness Hi Duxtarynn, You maybe able to get a superior pole fullness using your own breast tissue with a re-operative lift (not reduction) surgery. It does not make sense to get a reduction and then an implant augmentation if you can use your own breast tissue to fill out your superior pole. At Cosmoplastic Surgery, we have used the "Naturabra" technique that allows for your own breast tissue to act as an internal bra that supports the lifted breast while filling out the superior pole. We just reported our results of the past 18 years of over 700 breast lift cases at the 100th anniversary meeting of the American Association of Plastic Surgeons in San Diego in April 2022.

Answer: Revision Breast Surgery Although a proper examination would be helpful, it is entirely likely you would be a candidate for a surgery that reduces breast tissue, lifts, an is enhanced by the addition of an implant. You are correct in your thinking that reducing breast tissue and replacing it with implant will give a more idealized breast shape.

Answer: Bellesoma Method The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Best Wishes, Gary Horndeski, M.D.
