I have developed fairly prominent smile lines on either side of my mouth. Partly (I think) due to genetics and partly due to dehydration from acne treatments that I am currently addressing. I have always had the potential for smile lines because of the way my cheeks are shaped but I can’t help but think I accelerated the process with harsh acne treatments (that I am no longer using). What is the best treatment? It seems too early to get fillers but would dermarolling or lazers help me? Thank you
Answer: Reduce appearance of smile lines by doing Morpheus8 Morpheus 8 is a non-invasive skin and soft tissue tightening treatment that is consistently amazing in our patients. We use the Morpheus to exact fractional induced RF into the skin or tissue in order to tighten collagen and stimulate contraction of the tissue. For acne scars, it is quite useful to build collagen in patients who have atrophic scars but is not very productive for ice-pick scars which require TCA Cross. For tissue tightening, Morpheus is ideal for the face and neck and we often combine it with CO2 laser or Ultherapy to further enhance results. Chemical peels and skin resurfacing should be done at least 5 to 7 days after, but can be performed earlier, depending on treatment severity. Avoid exfoliating skin treatments before Morpheus and expect a period of 2-4 weeks for skin contraction to set in. Best, Dr. Karamanoukian Realself100 Surgeon

Answer: Improving deep wrinkles with botox/xeomin/dysport, fillers, fat, rf treatments, ultherapy, J Plasma, lasers, Sculptra A combination approach would give the most improvement. Deep wrinkles and smile lines can be improved with a combination of Botox and/or fillers for the lines themselves and to build anatomical support with Sculptra for collagen building. Ultherapy is a great skin tightening device, but works best in combination with other treatments like J Plasma, RF treatments, lasers, microneedling/PRP, salt facials, Cosmelan for skin tightening and improve skin tone. I recommend getting a formal evaluation with an expert. Best, Dr. Emer

Answer: Treatment for smile lines in a 21 year old Thanks for the question. It would be too early to use dermal fillers for "age related" lines but people can also have lines for lots of other reasons and dermal fillers can be very useful in helping with these. If they are fine lines then microneedling treatments (with or without PRP) can also be very effective and may help with any acne scarring that you have also. Good luck! Dr Chris Macdonald

Answer: Deep laugh lines in a 21 year old Deep laugh lines or nasiolabial folds (NLF) are a result of volume loss typically higher up in the malar Eminence, the cheeks, or under the eyes. As a result you need to first address those areas and then maybe fill in the nasolabial fold very lightly. This can start to in your 30s or 20s depending on your genetics and environment. You mention acne scarring and that can be treated with great certainty with micro-needling in combination with some great product like DefenAge or TNS Recovery. When it comes to fillers we've gotten great results with Restylane Lyft in combination with Restylane Defyne. Another newer filler to enter the market in January of 2018 is Versa. It's a great product and seems to last longer than a year. Radiesse can also give you great results. But, not every product is appropriate for every patient. It's a discussion you need to have with your doctor. My recommendation is watch some videos and learn about these fillers, their Pros and cons. You'll love what these can do to your face. Best from Boston, Dr. Onir
