Hello,You had very specific questions, and we would like to address those:We believe, based upon the photo, that your filler was placed too high up above the lip line and with too much volume.  This then creates an upward pull and attributes to a thick appearance.  When the upper lip gets too much or  more attention than the bottom lip, the final outcome appears less natural.  You may opt to have some of the filler dissolved to debulk the area and allow for a more natural shape without the lifting upwards.  We suggest you choose an injector (MD or nurse injector) that understands your goals and able to approach with a little more artistry.  The below link offers more feedback on mouth treatments using filler. 

Very sorry to hear that you didn't get the results expected. This could be because of the injection technique. Sometimes it can be fixed by adding little more product or you can completely dissolve and redo.
