Only considered tip plasty but consultation doctor suggested otherwise. I've been considering rhinoplasty to reshape my bulbous tip and narrow my nose bridge, however the doctor I consulted with also suggested goretex implant for bridge + alar reduction. Are these procedures necessary for me? Although it does look visually flat from front I think it's caused by the wide nose bridge, and narrowing it would help with the flatness, please let me know your thoughts.

For a taller bridge, DCF (Diced cartilage fascia) provides permanent results without the risk of warping or resorption of en bloc rib cartilage, and with a much lower rate of migration, infection, or extrusion than synthetic grafts. With a rhinoplasty surgeon experienced in this technique, diced cartilage wrapped in fascia is a consistent, reliable, long-lasting technique for creating dorsal augmentation with excellent results. While there are a variety of approaches and techniques employed by various surgeons, it is up to you to do your due diligence to find a rhinoplasty specialist in order to achieve the best results possible. Narrowing your nose bridge might make your nose look slightky more defined from the front, but I wouldn't expect a great effect by performing osteotomy only. For the best effect I would advise bridge augmentation with implants (silicone or cartilage) and tip plasty. Alar base reduction doesn't seem to be necassary.

I would recommend the more conservative approach of doing osteotomies to narrow your bridge as well as alar sill reduction to narrow your alar base. Along with tip refinement techniques this would give you a nice result without the risks of implants.
