I got my nipples pierced 6 months ago and they just have refused to heal and now get bloody, sore and have puss discharge as nasty as that sounds. I'm scheduled for a Breast augmentation in 4 weeks and are going through the nipple. Should I get on an antibiotic to get rid of the infection before surgery? If I can't get rid of it completely before surgery will I still be able to get my surgery? So worried. I know every doctor is different but give me your idea on what you would do.

The first step should be to take out the piercing if you haven't already- it is likely contributing to the inability to clear the infection. Then start warm water soaks with soap to help bring any purulent material to the surface. A course of antibiotics in the setting of an ongoing infection is something you should discuss with your surgeon, but I would not perform an augmentation on anyone with an active infection. I would likely also start antibiotics and delay your surgery until we are certain that everything has healed. It is always best to avoid elective surgery, especially one that involves placement of implants, until we are certain that the infection has cleared. I certain cases I would also perhaps recommend using the IMF (fold) incision to stay away from that area.Good luck.

Answer: Any infection should be cleared up before elective cosmetic surgery of the breast. Probably the best solution would be to remove the nipple ring and let the area heal before breast augmentation.

If you were my patient, my recommendation would be to have you remove your piercing and clear any infection from your nipple for several weeks before pursuing a breast augmentation. Placement of implants should be done in the cleanest environment possible. An infection in your nipple could seed an implant and cause at best an increased risk of capsular contracture, and at worse a very bad infection requiring antibiotics and the implants to be removed. It is unlikely your surgeon will allow you to have your surgery if you have an active infection.
