Chin augmentation with fat grafting, possibility and price? (Photo)

Question

I want to fix my weak chin at least a little bit but not just temporarily with fillers and definitely not with an implant, so I wanted to know if that was possible with fat grafting and what would the price be.

It's best to begin by consulting with a surgeon in-person to discuss your options. During your initial examination, your #chin will be evaluated along with the neck and jawline. A weak #chin implies a lack of chin support for the skin along the neck line. In younger individuals a chin implant alone is usually all that is needed to improve the appearance of the neck. Subsequently, it will also improve the overall #balance of the face and can create the illusion of a smaller nose. As a stand-alone procedure, chin augmentation can be performed in the office under local #anesthesia. For individuals with signs of #aging in the neck, a chin implant added to one of the recommended procedures above, will provide optimal results. Also, #liposuction under the chin is also another option your doctor can discuss. If you are not a candidate for chin or neck rejuvenation at the time of your #consultation, your board certified plastic surgeon can recommend alternatives to address your concern. 
However, there may be additional options to improve the appearance of your concern to improve the overall balance of your face. Such can be best evaluated in-person. Consulting with a board-certified plastic surgeon who specializes in #face, #neck and #chin procedures is important to ensure you are receiving the best advice, care, and surgical experience possible. A chin implant can give you more customization of your look. It can help projection, width and increase vertical height. Filler can improve projection and width but is less effective in increasing vertical height. Furthermore, the results obtained with implants are generally more dramatic than those obtained with filler alone. I would advise having a consultation with a board certified plastic surgeon who specializes in facial aesthetics to discuss best individual treatment plan.

It is possible to create good result with fat graft augmentation of the chin. However, it may take more than one procedure to achieve desired size/shape, 6 months appart. The beauty of fat grafting is that you can graft it in just about anywhere you need some volume.  In experienced hands and for good candidates, fat grafting to the face (including chin) is a great option as it offers many advantages over tissue fillers and facial implants.  It's much more forgiving and has less risks for infection and malrotation, as well as less scarring and less risks for nerve injury than chin implants (as well as quicker recovery).  We can usually be much more liberal with the volume of fat used for chin enhancement as opposed to tissue fillers for much more complete and comprehensive enhancement so it can actually be less expensive per cc than tissue fillers (which are usually guaranteed to go away and need for regular repeat injections are also guaranteed).  If the fat grafts survive the transfer process, it will be with you for many years (even decades)(something not possible with current fillers).  Based on your pics, I think you would be a great candidate as you have a good base to work with (your chin is not too small and your skin does not look too tight).  Fees can vary greatly from one surgeon to another and from one region of the country to another so I would recommend you see several board certified plastic surgeons with good experience with fat transfers in your area.  Best wishes.

chin augmentation is best done with an implant. I'm sure you have a good reason why you don't want implants. Fat grafting to the Chin is possible but it has nuances that are too long to discuss without a consultation. Cost depends on how much you need and some other factors. Best wishes.

The photograph demonstrates the significant underdevelopment of the bony mandible. A bony deficiency is best treated with a bony augmentation such as a solid silicone chin implant. Fat injections are very inconsistent and performed for soft tissue augmentation, not bony augmentation. Chin implants manufactured a large assortment of both sizes and shapes which is best determined at the time of the consultation and examination. They can also be custom carved when necessary to match patient's unique anatomy. Chin implants are usually placed through a small incision underneath the chin under local anesthesia. Hi, I have performed many facial shaping procedures, including Chin Augmentation with dermal fillers or silastic chin implants, for over 30 years.  From the photo, your chin is quite weak.  When the chin is weak, this creates an imbalance making the nose appear larger, the mid face top heavy, the lower face looks short, de-emphasizes the lips and allows early formation of a "double chin".  
 Proper placement of a silastic chin implant adds forward projection to the chin thereby creating harmony and balance to the lower face.  Using the same incision, liposuction can be performed to reduce the fat and further shape the neck.  Excess skin, from below the chin, can also be removed through the same incision.  
 I have found that placement of a silastic chin implant, through a small curved incision under the chin (also allows excess skin removal) to be very safe, quick, highly effective and far less invasive than a sliding genioplasty (requires extensive tissue dissection, bone cuts and placement of metal screws and plates to secure the cut segments of bone).  I perform chin implant surgery in 30 minutes or less, often using a local anesthetic alone.  In my opinion, you are a good candidate for chin implant surgery.   I have performed many facial shaping procedures using dermal fillers, facial silastic implants, liposuction and facelifts for over 30 years.  In my experience and despite its recent increase in popularity, fat transfer (fat injection) offers "far" less of a reliable and predictable volume for facial shaping than an off the shelf dermal filler or facial implant.  For that reason, I do not use fat to shape the cheeks, chin, lips or jaw line.  Tissue physiology is quite simple.  Tissue requires a blood supply in and out as well as lymphatic connections to remain viable and alive.  Once fat is removed from the body all of these things have been disrupted.  Just because the removed fat is mixed with PRP or something else doesn't make the blood and lymphatics magically re-appear.  The fat at that point is not living tissue which means that it's prone to being dissolved by the body (most likely in an uneven and unpredictable manner).  Injecting fat back into the face does not create the required elements to make the fat living tissue once again.  So the argument that fat is alive and viable in the face once it's been removed and re-injected makes no sense to me as a physician and surgeon.
 The other issue that I have with fat transfer is the lack of precision.  Fat is thick by nature which means it's not the same consistency as an off the shelf dermal filler.  Fat injections use an increased volume injected in an attempt to compensate for the volume loss that "will" happen.  This means a lack of specific shape and volume that simply can not begin to compare with the specificity of using a silastic facial implant of a "known" shape and volume.  In that regard fat offers too much of an unknown to make it a reliable and predictable method for facial shaping.  There's a significant difference between a 3mm and 5mm thick cheek implant.  You can imagine the magnitude of difference there is between retaining 60% of 25cc's of fat versus 35%.  In my humble opinion, I just don’t see how fat could possibly be used to precisely shape facial features?
