Hi, I have performed many facial shaping procedures, including Chin Augmentation with dermal fillers or silastic chin implants, for over 30 years. From the photo, your chin is quite weak, there is excess fat and skin below the chin (in the neck) and the back portions of the jaw line are weak. When the chin is weak, this creates an imbalance making the nose appear larger, the mid face top heavy, the lower face looks short, de-emphasizes the lips and allows early formation of a "double chin". Proper placement of a silastic chin implant adds forward projection to the chin thereby creating harmony and balance to the lower face. Using the same incision, liposuction can be performed to reduce the fat and further shape the neck. Excess skin, from below the chin, can also be removed through the same incision. I have found that placement of a silastic chin implant, through a small curved incision under the chin (also allows excess skin removal) to be very safe, quick, highly effective and far less invasive than a sliding genioplasty (requires extensive tissue dissection, bone cuts and placement of metal screws and plates to secure the cut segments of bone). I perform chin implant surgery in 30 minutes or less, often using a local anesthetic alone. In my experience and humble opinion, you can certainly have chin implant surgery, as described above, resulting in an improved chin projection and balanced lower face. Selecting the proper implant size, shape and thickness is "key" to obtaining those "natural" results and avoiding masculinizing the chin and lower face. When augmenting the face with facial implants select a surgeon with significant "experience" in silastic facial implant placement. While the back portion of your jaw line is weak, over correction with a jaw implant or too much filler volume can result in a masculine jaw line. The cheeks are not shown in the profile photo, so this is a generalization. If the cheeks are flat or concave in the front portion of the cheeks, this creates an elongated oval or rectangular shaped face. Following my beauty principles, women look the most feminine, youthful and attractive with heart shaped faces. Heart shaped faces have cheeks that are full and round in the front. Cheek augmentation with a dermal filler or using cheek implants for a permanent enhancement will create full, round cheeks that will feminize the entire face. Having full round cheeks in the front and a heart shaped face is essential in women who have prominent jaw lines to prevent the face from appearing relatively masculine. In your case, if you decided to have precise placement of a dermal filler to enhance the back portion of the jaw line, it would be important to make sure that your cheeks were also ideally full and round in the front.Hope this helps.
