The best option and most economical is for you to simply lose the weight with diet and exercise. I realize that that sounds obvious and also sounds like a lot of work. But it really is your best option. Liposuction alone will likely result in the skin laxity that your SmartLipo doctor has alluded to and gluteal lifting procedures are going to be expensive. They also have a relatively high complication rate. You will be much happier if you can do most of the work by yourself.

I agree with the opinion of the surgeon who stated that sagging might be the byproduct of liposuction. You might find that the buttock will look smaller by suctioning the peripheral areas avoiding direct suction to the buttock itself.

Dear Zeepan,           Thanks for submitting your pictures. I would like to recommend to you a different approach than the other answers. From your pictures it appears that you have skin and muscles laxity of your abdomen. A tummy tuck will do the following for you : 1. Flatten your stomach. 2. change your posture which will help you with your back problem. 3. Very good chance that you will lose significant amount of weight due to the muscle tightening which will reduce your hunger and food consumption. 4. The weight loss will be generalized and so it will reduce the size of your butt too. 5. Your self esteem should improve due to your improved looks and sense of well being. The tummy tuck will probably cost you more , but it is worth it because of the above mentioned reasons while the liposuction might give you sub optimal results that will reduce your self esteem. You are investing in yourself, so invest wisely. My suspicion is that your buttocks are not the issue as much as the tissue around it. Two things can happen with liposuction in the area around the buttocks. 1.  The butt itself is going to look better, more shapely and smaller. 2.  The area around it is going to be thinner accentuating the shape that you want but looking better and fitting better into clothes.A second option would be a gluteoplasty or butt lift. This is different from the Brazilian Butt Lift which uses liposuction and fat grafting. With a Gluteoplasty, incisions are used to provide access to the deeped tissues which are sculpted. The area around it is thinned with liposuction and excess skin can be removed. This is a good bit more invasive than the liposuction option and more costly in most centers.Best,Dr. Pyle

you may need to get a combination of liposuction and buttock lift.  Your photos are limited, but your choice is to get either liposuction alone with the thought that you may also want to do buttock lift if the liposuction does not achieve the results you want.  Look at the before/after photos of buttock lift on the surgeons website.
