Yes, everything you say makes sense. The breasts are very 'mildly' tuberous with enlarged areolae and slightly pyramidal shape but without true areolar herniation. I would be hesitant to do an 'areolar reduction' because this would probably not lift the breast as much as expected and it might even 'flatten' the breast contours. And then, a more significant mastopexy of another type might effectively reduce the size even if the shape is better. If you are expecting to have children anytime soon, it would be best to wait. If not, I might do a very conservative augmentation, and then if necessary stage a mastopexy. Hope this helps.

You do not have tuberous breasts based on these photos but a variety of narrow base breast. A typical correction is similar to correction of tuberous breasts, reshaping of parenchima, areola reduction and lift. Good luck.
