The common answers of botox for the depressor muscle or filler will be brief and partially correct the concern. A correctly sized chin augmentation will release the ligaments pulling the corners of the mouth down and more importantly produce a better jawline. This is permanent and rarely but easily removed.

Its not a long last treatment, but I would recommend trying Botox injections into the DAO (depressor anguli oris). Some people have a hyperactice DAO muscle which can be the main reason the mouth corners pull down. Botox can help the actual problem and help prevent this from getting worse as you get older.

As you'll see from the various answers, you have a few options, different price points, down time, discomfort and duration for each of them. In your case I'd try some mid-cheek fillers and DAO Botox injection and see if that gives you the result you desire. If insufficient, a surgical correction (e.g. chin implant) may be considered.

I would advise a neuromodulator injection (such as Botox) to the DAO (Depressor Anguli Oris muscle). Neuromodulators block signals from nerves to the muscles and because of this the muscle is unable to contract. This is often subtle when done alone. If combined with a filler, it will have a more dramatic effect. Filler will help restore the loss of volume.  The results can be fairly long lasting, typically 9-12 months.

I like treating with Sculptra injections in my younger patients as it revolumizes/ recontours the face and it stimulates new collagen which helps with firming and mild sagging. In addition, adding Botox including to the depressor muscles of the mouth could help. Best to you.

A small amount of botox into the Depressor Anguli Oris muscle will help somewhat. However, your biggest problem is the fact that your cheek and lower eyelid area is extremely hollow and that results in the descent of the central portion of the face. Restoring volume in the upper face with the use of your own fat will result in better facial proportions and appearance as well as elevation of the area around the mouth.
