A rhinoplasty specialist will be able to comprehensively address the issues you have with your nose. Alar base modification is an effective way to reduce the flare and/or width of the nostrils. Excess width can be reduced with sill incisions, excess flare with Weir incisions, and excess flare + width addressed with a combined Weir+sill incision. However, there is an important relationship between the proportion of the width of the nostrils relative to the width of the tip, and if the nostril width is reduced without addressing the width of the tip - it will appear even more bulbous in comparison. Depending on the thickness of your skin, you may need some degree of tip projection to achieve a refined tip. Without projection it will be difficult to see the changes made underneath the skin. If you want the best idea of what is possible for your nose, do your due diligence, find an experienced surgeon, and have him/her evaluate you in person and discuss your options. @drdonyoo
Bulbous nose is usually a term that refers to the shape and size of the tip. Ala are the wings that are alongside the tip. You could have a situation where you can have both a bulbous tip and large flaring wide ala. Alarplasty will only address a wide large and flaring ala, while a tip-plasty could address a bulbous wide tip.
Every time you bring the nostrils in to reduce flaring, you make the tip appear slightly more prominent! So it has to be done with that in mind. In your particular case, you would be best served with a tip graft to increase the tip projection and produce tip narrowing. That would allow for alar...
Your photo is somewhat limiting because it doesn’t show the nostril openings. To me it spirals that your nostrils were not narrowed enough. If you do get a revision the nostrils should be narrowed if possible. This will improve the overall look
Aylar plasty: an alar plasty can always be performed as a separate procedure if you are happy with the tip and rest of your nose. Bear in mind that the nostrils should bear a Pyramidal type relationship to the tip when viewed from underneath which is called the basil view. you did not provide a...
The term "alarplasty" usually means bringing the attachment of the nostril to the face either inward or outward (to correct flaring or a prior alarplasty). What you are desirous of is technically also an 'alarplasty", but doesn't involve moving the attachment inward or outward. It is a common...
There are 3 versions of what is meant by "higher". 1-Higher attachment of the nostril to the face. 2-Higher height and alar groove to that nostril. 3-Higher rim to that nostril. The last two are much easier to correct than the first! In any case, the surgical correction should be performed in a...
Alar base surgery: Alar base surgery can always be done as an isolated procedure if that is all that bothers you. If alar base surgery is done alone, it could be done under local anesthesia in the office. However sometimes what makes a “mean look“ is a downward drooping of the tip as well as the...
