I have had 350cc round over the muscle implants started a deflated c cup, had a lift with inplants. they look saggy from the side I’m worried the lift hasn’t worked

It is possible for breasts to look good from the front but still appear low or saggy from the side. The side view often shows the relationship between the implant, breast tissue, nipple position, and lower pole skin more clearly.
With 350 cc over-the-muscle implants, if the original breast tissue was deflated and loose, the tissue can still drape over the implant or settle into the lower pole. This does not always mean the lift “failed,” but it can mean there is residual skin laxity, lower pole heaviness, or not enough internal support.
Over-the-muscle placement can also make implant position and tissue coverage more visible, especially if the skin envelope is stretched. If the implant is sitting high while the breast tissue hangs lower, this can create a “waterfall” type appearance from the side.
The timing matters. If you are still early after surgery, swelling and settling can change the shape. If you are several months or more post-op and the side profile remains bothersome, revision options may include tightening the lower pole, revising the lift, adjusting the implant pocket, changing implant size/profile, or adding internal support.
My advice would be to review your pre-op photos and current side photos with your surgeon. Ask whether the issue is normal settling, residual ptosis, implant position, or loose lower-pole skin. That diagnosis will determine whether observation or revision is the better next step.

Your breasts are too low on the chest wall and the weight of the implants is causing them to descend. At this time, I recommend explantation and lift using the Bellesoma. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers or small implants can be placed if additional volume is desired.

Thank you for your question, When 350cc round over-the-muscle (subglandular) implants are combined with a mastopexy (breast lift), a side-profile "sagging" appearance usually happens for one of a few specific structural reasons. Because your implants sit above the pectoral muscle, their weight is supported entirely by your native skin envelope and internal tissue stitches rather than the muscle wall.

By placing the implants over the muscle they are being completely supported by the the skin.  The weight can sometimes be too much for the skin to hold up.  Switching to an under the muscle approach and taking up extra skin on the lower pole may give you a more lifted look.  I encourage you to visit with a board certified plastic surgeon near you to learn more.

Hello, thank you for sharing your photos and surgical history.
From the side view, there appears to be some remaining breast tissue below the implant, creating a contour similar to a double-bubble deformity. In this situation, the lift does not appear to have provided enough support or elevation to the breast tissue.
A revision would likely involve lifting and reshaping the breast tissue a little further, while also lowering the implant slightly so that the implant and the natural breast sit in better alignment.
The implant size itself appears appropriate, so changing the implants may not be necessary. Unfortunately, this type of contour problem is unlikely to correct itself with time and would generally require surgical revision for a meaningful improvement.
An in-person examination would still be necessary to assess the implant position, breast fold and skin quality before deciding on the exact revision plan.
