Allergan breast implant - capsular contraction. How do I file a claim with Allergan to have my implant replaced?

This paperwork can and should be submitted by your surgeon.  Patients who fall within the warranty period for implants may be provided with free implant replacements as well as money toward the cost of surgery (fees associated with anesthesia, surgeon, facility use etc.)

should be done by your surgeon who is doing your surgery.  Your old records will be needed and you will likely have to pay for your procedure up front until the warranty is paid out.  If you go back to your original surgeon, they may allow your to schedule without full payment if you are willing to sign the check, if applicable, over to them.

Sorry to hear that you have a Baker Grade 3 capsular contracture.   Your plastic surgeon can file the paperwork with Allergan.  You will need to have information on the implants that were placed including the serial number.   Your previous plastic surgeon can provide this information if you no longer have the implant information. 

The doctor of your choice will file the claim for you, and will deal with all the required paperwork.

 Fairly recently, implant companies began providing warranty is for capsular contracture. This is a new thing and if your implants were done many years ago you probably have no warranty for that at all. You should check with your surgeon and check with the implant company to see if you're covered by this or not. If you don't have a warranty you'll have to cover the cost of this out of your own pocket.
