A huge goal of mine getting breast augmentation was to have breasts that sat perky and high. My boobs now look like bigger versions of my natural boobs and I still have to wear a bra with tank tops etc. There seems to be very little volume on the top of the breast with much more volume at the bottom. Have they bottomed out? Or should I be looking at getting a lift?

I would suggest going down in size, keeping your implants behind the muscle, and perhaps another lift to tighten some breast tissue. Measurements are needed to be sure, but just going smaller and adjusting your implant pockets may be all that is needed. Your implants are sitting too high.
With your anatomy, you will not have narrow cleavage. However, a lift can be done to raise your nipples and breasts. I would not go larger for you may sag again. Your surgeon tried to use a wide profile implant to narrow the space between your breasts. However, the actual base of your breasts...
You can have the implants placed in front of the muscle, but you may still have some animation deformity and you may see the edge of the implant. Long term, implants do better behind the muscle in most women. You can go down in profile and go with a smaller implant for less fullness.
