I would recommend a release of the capsules in your lower poles so that your implants can drop and fluff more attractively, and an areola reduction.

Your areolas are too big and the implants are too large. You have abnormal upper pole fullness. You need smaller implants and a circumareola approach to decrease the size of the areolas. At the same time, implant exchange can be performed.

I would suggest going down in size, keeping your implants behind the muscle, and perhaps another lift to tighten some breast tissue. Measurements are needed to be sure, but just going smaller and adjusting your implant pockets may be all that is needed. Your implants are sitting too high.
