I got my breasts done in October 2020 (saline 255/275cc under the muscle) I feel as if my breasts dropped too low/ are bottoming out. They're farther apart now and when I lay on my back they go to the sides. My nipples seem to be too high and far apart as well. Its uncomfortable going bra less & wearing underwire bras, I usually only wear sports bras. My scars also are not on my breast crease they're a little above, do I need to get a revision?

Because of your large implants, they have stretched your tissue and have dropped some. I would suggest going smaller and tightening the lower pocket. If you stay the same size or go bigger, you will have the same problem perhaps sooner.
I would suggest going down in size, keeping your implants behind the muscle, and perhaps another lift to tighten some breast tissue. Measurements are needed to be sure, but just going smaller and adjusting your implant pockets may be all that is needed. Your implants are sitting too high.
With your anatomy, you will not have narrow cleavage. However, a lift can be done to raise your nipples and breasts. I would not go larger for you may sag again. Your surgeon tried to use a wide profile implant to narrow the space between your breasts. However, the actual base of your breasts...
