Which is better to change the lip shape without concern to increase volume? Filler or thread lift? (Photo)

Question

I’m not happy with my upper lip shape. I’m not concerned about increasing the volume/plumping, but would like to change its shape to be more feminine. They look too triangle-shaped and lacking definition on the sides. I’ve heard of PDO thread lifting to enhance lip definition. I’ve attached photos of my lips and a photo of my ideal lips. Any suggestions whether lip fillers would be best vs thread lift to achieve better results?

Most doctors will start with one-syringe to avoid overfilling. These fillers, (Volbella, Restylane, Juvederm) are good to create a symmetrical and full lip. PDO NovaThreads can also be used to improve shape and also provide lift. Fat or implants are a permanent alternative. I recommend getting a formal evaluation with a facial sculpting expert. Dear Sheila8383,
I perform a lot of lip injections with fillers such as juvederm and it works great. Many times I need to put more in one side than the other to attempt to even out the vermillion border (The red line of the lip). This helps provide more symmetry at times.
For patients looking for a more long lasting result, I recommend fat grafting. That is, using your own bodies fat to make your lips naturally look more shapely. The bonus is that the results last many many years.
In general, I avoid using permanent prosthesis because I find that the lips are such dynamic and sensitive structures, the end result is not natural.

Lip fillers by definition will primarily add volume. They are not particularly good at lifting the corners of the mouth.If you have no interest in adding volume then I don’t think fillers are going to achieve what you’re looking for. The biggest difference between the picture of you and your ideal picture is the lifting of the corners of the mouth.Achieving this is not easy. You can try having Botox injections in the depressor oris anguli muscle. My experience both personally and professionally of this treatment is that it doesn’t lift the corner as well as described by others. A thread lift is an option but those can lead to sticky situations that are complex and difficult to manage if things don’t turn out the way you hope.You can try letting someone work with fillers. At worst the results will eventually simply go away.If you like it then you can continue with that treatment and that provider.Another option is to recognize you have nice looking lips and to leave good enough alone.I think you’re going to find it difficult to find someone who can successfully create the specific change you are requesting.Best, Mats Hagstrom MD
