I scheduled a septoplasty with an ENT surgeon, and they let me know that they are able to bring in a plastic surgeon if I’m wanting both. My insurance would cover the septoplasty portion. I’m not sure it would be wise for me to do both now due to finances, so I was curious if I would likely end up spending more in the long-run if I schedule a rhinoplasty in a year. Is one recommended over the other?
Answer: Septoplasty and Rhinoplasty Hello, Great question. It is best to do both at the same time because a good rhinoplasty requires cartilage from the septum. The most important part here is going to be to choose the correct rhinoplasty surgeon. Rhinoplasty is a very complicated surgery and is different from all other on many levels. I do a lot of revisions and often try to understand where patients went wrong on their first surgery. Often times it was that rhinoplasty was added on as "just another procedure". Make sure the surgeon specializes in rhinoplasty. Sit down and review the changes, preferably using computer imaging. Understand what the surgery entails. Rhinoplasty is not a simple, remove the hump, cut the cartilage and call it a day type of procedure. We have to make sure we change things precisely and support them adequately. I have added a video for what to look for in a consultation, but make sure to check out the other videos as well, especially regarding the anatomy of the nose ... this will help you become a part of the conversation when discussing your nose. Additionally, if grafting is required for your rhinoplasty, it is a valuable source of cartilage (that would likely otherwise be removed during a septoplasty. Most rhinoplasty surgeons would consider doing a rhinoplasty after a prior septoplasty had been done as a revision surgery. If possible, it is best to have this done by a surgeon trained in both functional and cosmetic rhinoplasty who can accomplish both in the same setting.

Based on our experience over 25 years, you would be most likely to achieve a functionally and cosmetically favorable result if the same surgeon addresses both your Airway and your nasal aesthetics at the same time. Since rhinoplasty is considered one of the most difficult facial plastic surgical procedures, one would think that someone who is able to perform rhinoplasty would be able to correct your deviated nasal septum as well. Additionally, we nearly always use somebody's nasal septal cartilage for nasal tip reconstruction and support. Hope this helps! Dr Joseph

Answer: Likely yes The #1 reason why we'd love to do a rhinoplasty and a septoplasty at the same time is that for the rhino to happen, we need cartilage, and what better source of cartilage is there than cartilage already from your nose/septum? If you do the septo first and then do the rhino in a year, it is very likely that you will not have enough cartilage in which case you may have to get cartilage harvested from somewhere else (ear/rib), which will incur an extra cost and recovery to you. At the same time, I would NOT encourage you to do a rhinoplasty unless you were 100% sure you wanted to do it because there is a lot of healing involved with rhinos, both physically and mentally, so it's not something you should do "Just because" you are getting a septo done too.
