Although you’ve had a chin implant, if the sagging in the lower face and the downward pull at the corners of your mouth persist, I would recommend a combination of lower facelift and anterior neck lift to address these concerns effectively. A lower facelift helps to redefine the jawline and lift the jowl area, restoring structure and youthfulness to the lower third of the face. The anterior neck lift, on the other hand, specifically targets the front of the neck by tightening loose skin and muscles, creating a smoother, more defined transition between the face and neck.
In addition, to restore lost volume and soften any hollow or deflated areas — particularly around the chin and cheeks — I often recommend fat grafting. Using your own purified fat, this technique replenishes volume in a natural way and can significantly improve overall facial harmony and skin quality.

In general, the laxity along the jawline and around the corners of the mouth, as well as the position of the corners of the mouth, is most effectively addressed by a deep plane facelift. This technique allows for the mobilisation of the deeper structures that contribute to cheek contours and support, resulting in natural and harmonious contours.

Additionally, the deep plane facelift also improves the appearance of the neck. However, depending on your personal preference for the final neck contour, a neck lift intervention may complement the results and better align with your aesthetic goals.

The downwards corners can be corrected with a lip lift. A lip lift does 4 things: shortens the distance between your nose and upper lip, increases the mucosa of the upper lip, increases the incisor show and pulls up the corners. You are an excellent candidate for a lip lift.

I'm a bit puzzled about why a chin implant was performed to improve marionette lines (the downturned folds at the corners of the mouth). Frankly, I am also puzzled why a chin implant was opted for when the use of a robust HA filler could have been used here, as well, to smooth the uneven bumpiness. Although the photos provided do not allow for adequate evaluation, it does appear that the use of a robust HA volumizing filler for the cheeks, appropriately injected, would establish a oval face and Ogee Curve and secondarily improve the smile lines and likely the marionette lines, as well.
You would be wise to consult an experienced, board certified cosmetic dermatologist filler expert. You might be very pleasantly surprised that in about a thirty minute treatment session, you can have immediate results with little to no significant downtime. Hope this helps and best of luck.
