I'm 19 and considering getting rhinoplasty. I have come to accept my "hump" but I'm hoping to refine my bulbous nose. From certain angles it looks like I don't even have a nose tip and that it is just a box. Also what might be the price point for tip rhinoplasty in Massachusetts? One of the photos included is my rhinoplasty goal.
Answer: You are a good candidate, but you have a difficult nose to work on. See the "Web reference" link, just below my response. I made a computer morph, and an animation of the morph, to show the changes that are possible for your nose in truly expert hands. I narrowed the tip of your nose, elevated that columella that droops somewhat, and adjusted the bridge of the nose, too.You did a very good job on your morph of the profile view -- elevating the columella and adding a little projection to the tip.I took a look at some of your other posts, too. *Absolutely, absolutely do not let someone do a "columellaplasty," whatever in the world that is. Your columella is the same cartilage as the tip of your nose, and an attempt to only work on the columella would do great damage to your tip, and you'd hate it. You dont' have a nose that can tolerate just a little tweak, or a limited operation.You should understand that the changes I demonstrated in the morph, and the changes you would need if you have surgery, involve modification of the tip cartilages, the hardest part of working on a nose. Those changes require advanced techniques, techniques that most plastic surgeons cannot handle. Be sure to read the section in the "Web reference" link on how to stay out of trouble while searching for a rhinoplasty surgeon.

Your surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. Cartilage grafts may also be used to help improve tip refinement. The most important aspect is to find a surgeon you are comfortable with. With good surgical technique, you could have an outstanding result. When patient's have a dorsal hump, it is not possible to only perform tip surgery. A bulbous nasal tip is addressed with suture techniques and a conservative cartilage removal applied to the lower lateral cartilages of the nasal tip. It's important that all the components of the nose balance with themselves, and the new nose balances with the patient's facial features.
