Question

Does my under eyes have bags or malar bags or fluid retention? Or something else ? I would like to treat the line and puffiness . I am not interested in fillers quite yet . Any other treatments I would consider.. When I don't smile you can see the line from my tear through to my cheek. When I smile it bulges up it causes puffiness and wrinkles . I tried lymphatic message and eye bag creams. I have used a retinol for years . . Only seems to be getting worse and not any better..

Try a good eye cream. The $300 eye creams are not any better than the $30 eye creams so don't spend too much money. When you are ready, these are beautifully handled with the hyaluronic acid filler Restylane. Less is more.

The best treatment for this area is filler which will help disguise the contour irregularities.  As you mentioned you are not ready for that you can consider Morpheus8 treatments as well as medical grade skin care products to help keep skin tone and healthy.

What you have is generally called a tear trough. What you have is one caused by a double orbital ligament where the medial component extends down the malar bone. As you lose volume and the superficial tissues descend a depression is formed. The best treatment is to refill the area by injecting either fat or a filler. The filler or fat needs to go directly on the bone below the ligament. If you use a filler it should be heavy like Juvederm Voluma (which I have had last 5 years here) or Radiesse (also long lived here, though usually only 1-3 years). You can also move fat into the area surgically (extended lower blepharoplasty or mid-face lift), though I would not recommend that.
