My eyes are large but have a negative canthal tilt that makes them look sad, tired and droopy. Are there any procedures that can raise my canthal tilt to a more positive one? I’m a teenager so this is a genetic feature, and I’m not trying to seek out anything anytime soon, but I really don’t like this feature and wouldn’t want to stick with it long term if it can be changed.

Yes it can be permanently changed. It requires lower eyelid retraction surgery with canthoplasty. Important to have detailed analysis and discussion in consultation with an expert.

While oculoplastic surgeon are certainly trained to perform lateral canthoplasty, you actually need a surgeon who understands how to aesthetically control the lower eyelid. You actually also have an upper eyelid ptosis issue. When your lateral canthus is properly positioned, your upper eyelid ptosis will be worse and will need to be corrected surgically. This combination of procedures is often performed at the same surgery. There is no substitute for an in person consultation. Your eyes can be your best feature.

Hello! Yes, there is a surgical fix for what you have: bilateral (both lids) lower lid retraction (downward displacement of lower lid edge such that there is white sclera showing below the "6 o'clock position" of the iris/colored part of the eye) with lateral canthal angle inferior displacement (downward position of the outer corner of the eye relative to normal). In nearly all situations, all this can be corrected with bilateral lateral tarsal strip procedure (aka Kuhntszymanowski procedure). It is a procedure done extremely commonly by oculoplastics subspecialists, like myself.
