The short answer to this often asked question is, if you were my patient, I would think Benadryl would be an excellent treatment for a topical rash. I might also recommend a topical medicine such as Betamethazone cream. Since you aren't my patient, I highly recommend you call your personal plastic surgeon's office for the answer to this appropriate question.
If you have developed a rash you will want to discuss the situation with your surgeon. It may be that you will need to stop wearing the compression garment or put a barrier between your skin and the garment. Your surgeon is the best one to advise you on the next steps to take.

It sounds like you are having an allergic reaction to your garment. This is not unusual since you wear it continuously. Try other garments as well as wearing a cotton T shirt under the garment. Never take medication without permission from your surgeon. Contact him regarding the rash.

It may be that an ultrasound is needed to determine if the area in question is perhaps not a seroma. It could be swelling, which would explain the inability to aspirate it. It could potentially be a hematoma as well. An ultrasound should reveal more information.
