6 years post face and neck lift - would a revision allow for smoother results? (Photo)

I had a lower facelift/necklift done about 6 years ago. It never looked smooth. An attempt was made to fix it by the surgeaon but it did not work. I am attaching a photo and would appreciate any reccomendations. Thank you

It is very likely that a revision of anterior neck to release scar attachments and reshape and remove platysma muscle bands would be sufficient. This can be done through already existing incision under the chin and local or twilight anesthesia. Good luck.

Hi DFwest, thank you for sharing this, and I am sorry the result has not settled the way you hoped.

From your description, a lower face and neck lift that never looked smooth and one revision that did not help, the usual issue is the deeper layer rather than the skin. If the first operation tightened skin without fully repositioning the deeper structural layer and the platysma muscle in the neck, you can be left with irregularity, banding or a tethered look that a skin-only revision will not correct. I was not in that room, so I cannot know exactly what was done.

Six years on, the tissues are stable. That is actually a sensible point to assess a revision properly rather than rushing. A deep-plane approach treats the deeper layer and the platysma directly, which is often what smooths a stubborn neck when skin tightening alone has not.

This really needs an in-person examination with your photos and full history before anyone can advise you honestly. I only operate on faces and see a good deal of revision work, and sometimes the honest answer is that a smaller, targeted adjustment serves you better than another big operation.
