I have previously had maxilla widening and advancement which helped balance out my midface and to correct my bite, but I'm still not happy with the overall balance. I am considering rhinoplasty to decrease the height of the profile as well as fix a deviated septum, but I worry that will also make my chin look extra extrusive.
Answer: Facial implants and chin reduction surgery Thank you for the photos. A combination of chin reduction surgery and facial implants would likely provide the best results. You show significant temporal hollowing and I would recommend temporal implants to provide a wider upper third of your face and enhance the "triangle of beauty". For the lower jaw, both custom mandibular angle implants and chin reduction surgery would provide a more feminine appearance. Lastly, Following the above surgeries, additional soft tissue of the chin itself may need to be removed from a skin side approach. I would only consider this following the bony work.

Answer: Facial proportion and balance Thank you for your question. I would focus on three main areas to address your concerns. First, I suggest rhinoplasty to help provide improved balance and symmetry to the face. The excessive volume of the tip of the nose and the deviation of the nose draws the eye to the mid face. Second, cheek augmentation would provide improved balance. You have a very thin face and adding volume to the cheeks will help improve the balance. Third, the chin is long vertically and reducing the chin height will help provide better proportional balance. Please let us know if there are any other questions or concerns!

Answer: Jaw Reshaping You have a high and narrow jaw angles combined with a long and wide chin. The combination of chin reduction through intraoral T-shaped osteotomies (vertical and width reduction) combined with jaw angle implants that provide some vertical and width increases would provide favorable aesthetic changes to your lower face. Rhinoplasty also has value, although not as significant as lower jaw reshaping with correction of the deviation and tip narrowing.

Answer: Facial balance and jawline You have a vertical growth pattern so ideally you would augment the cheekbone area/midface to balance the jaw and nose. The high slope of the jawline emphasizes the chin prominence with a more narrow jawline; so a chin vertical change could also be considered; given that you have tried to augment the width of the lower jaw

Answer: Facial Balance Thanks for the interesting question. We can't make specific recommendations for an issue of this complexity without a full consultation and examination, but you may also want to consider a fuller midface with cheek implants and definitely consider a chin reduction (not a setback of the chin).
