I’m a 46-year-old woman who had an upper and lower blepharoplasty in October 2025. I’ve had hereditary under-eye bags for most of my life, and I do appreciate my surgeon’s work. I look noticeably better overall. That said, I’m wondering if the result may have been a bit conservative, particularly in terms of fat removal under the eyes. I still see some fullness that concerns me. I’d really value professional opinions on whether this looks within the normal range of outcomes, or if it might be worth considering a revision. At October 2025 surgery, you're now around 7 months out. This is right at the inflection point where tissues are settled enough to evaluate, but where residual healing is often still improving for another 3 to 6 months.
Why the 6 to 12 month mark is the typical 'wait and see' zone:
Tissue healing continues until 12 months for most patients. Visible swelling improves dramatically in the first 6 weeks, then more subtle settling happens between 3 and 12 months. What looks like a problem at 6 months sometimes resolves by 12.
Scar tissue maturation. The internal scar tissue from lower bleph remodels significantly between 3 and 12 months. Asymmetries you see now often improve as scar matures.
What to evaluate NOW (specific concerns):
Lid retraction or scleral show (white showing below the iris). This is the most concerning post-bleph finding and rarely resolves on its own. If present, revision planning should start now, not later.
Persistent visible bag or asymmetric fat. If one side clearly has more residual lower-lid fat than the other, surgical correction is reasonable to discuss.
Step-off at the lid-cheek junction. A visible groove or contour irregularity here often improves with massage and time, but if it's not improving by 7 months, evaluation is warranted.
Numbness or sensation changes. These usually improve through 12 months. If significantly worse than expected, evaluation is helpful.
What to wait on:
Mild puffiness or fullness that fluctuates. Often residual lymphedema that improves with time, lymphatic massage, and gravity.
Subtle asymmetries. The two sides of the face are never identical. Some minor asymmetry at 7 months often equalizes by 12.
Skin quality changes. Pigmentation and texture improve through 12 to 18 months.
My honest recommendation:
If you have any of the concerning findings above (lid retraction, persistent asymmetry, contour issues), see your original surgeon now for evaluation. They may want to start revision planning even if the actual revision is months away.
If your concerns are more subtle, wait until the 12-month mark before considering revision. Many patients who feel they need revision at 6 to 7 months change their mind by 12 months.
When you do see the surgeon, bring photos from various time points (pre-op, 1 week post-op, 1 month, 3 months, current). This helps trace the healing trajectory and decide what's improving vs static.
Final note: any revision should ideally be at 12 months minimum from the original surgery, and ideally with the same surgeon if your relationship is intact. Going to a different surgeon for revision is sometimes necessary but adds complexity.

Hi there thank you for your question and for sharing your photos! I see you have had a prior blepharoplasty. I think the surgeon had done a very nice job and has significantly improved the contour and shape of the lower eyelids. While I see your concern about some residual “puffiness” of the lower eyelids, it is quite normal to see this in the general population. It may be better to wait some time before considering a revision down the road.
