Uneven after exchanged 450cc (filled to 585cc) saline for silicone 700cc, Mentor ultra, advice? (Photos)

I understand your concern. However, without a proper assessment, it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

You're photographic record clearly shows you did not have symmetry prior to surgery, remained to have asymmetry with your saline implants, and now have approximately the same asymmetry. The only difference is that the size of your breasts kept growing, magnifying your pre-existing asymmetry along the way. The problem is one breast has a larger soft tissue envelope. To give you the benefit of the doubt, one implant may be sitting a smidge lower, but most of what you see you came with from the start.

In your before photograph, your breasts were low on the chest wall and you needed a lift. Implant alone will not provide a satisfactory result. At this time, I recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers or small implants can be placed if additional volume is desired.

One side is a little larger, but you were not even to start. If that bothers you, please discuss this with your surgeon. Your initial surgery appears that your surgeon selected large implants to avoid a lift. However, volume will not replace a lift.

Your breasts were asymmetric in shape, size, nipple position and fold origin from day one. I inform my patients that sometimes when you enlarge the breasts it will enlarge the asymmetries. In my opinion, your breast implants are too large for your body habitus.

When saline implants rupture, the body will usually absorb the water fairly rapidly with fairly immediate loss of volume on the ruptured side.
When this happens, the scar tissue surrounding the implant called the capsule begins to shrink.
In order to have room to fit a similar size implant implant replacement should be done as quickly as possible.
If patients wait or want larger implants than the capsule needs to be opened.
There are too many variables involved in your outcome to give you a clear assessment and description of why your results look the way they do.
In your case, I suggest you in person the second opinion consultations to get a quality assessment and better understanding of potential treatment options at this point.
To get a proper second opinion, I suggest patients come prepared to bring with them a complete set of proper before, and after pictures of each of your previous procedures and copies of your previous operative reports.
These should all be available for your current provider if you request them.
Anytime, excessively large implants are used the chance of having undesirable side effects, complications and need for revision surgery will always be higher.
