How to get rid of these bumps next to nose? (photo)

48 yrs.Wondering what these bumps are next to my nose? They have begun protruding over past few yrs. Not noticeable unsmiling but can be seen with big smiles or expressions. I got under eye filler 5 yrs ago, unsure if that’s what it is? But the bumps feel hard and bony. I hate them, they make me look like I have bags (I normally don’t) and cause weird shadowing on the face. Sorry for the bad photos, tried to show where they were. Maybe this is just from thinning skin? Help!

If those bulges feel firm, perhaps this is filler. Since they are symmetrical on both sides, my initial impression was that this could just simply be nasalis muscle on both sides. Please consider meeting with several reputable facial plastic surgeons for a definitive answer. Sincerely, Dr Joseph

Fillers persist in the face much longer than many of us realize. As the products sit in the face, there character and the manner the hold onto water changes. The issue you are concerned with may or may not be caused by retained, aging filler. Imaging such as ultrasound or MRI is being offered but these methods are not sufficient sensitive or specific to be particularly useful clinically. That means you are wasting your money having these studies done. It is far better to simply get enzyme service. If your issue resolves then it was caused by retained filler. If not, something else is causing the issue. There is no substitute for a detailed, in person assessment.

From these two limited photographs, the bumps you are referring to are dynamic skin bunching relative to your smiling muscle. There's no way to get rid of these dynamically. A cosmetic rhinoplasty can accomplish narrowing your bridge line of the nose, but you'll still have those bumps when you smile, even after nasal surgery.
