Dear Fared, Thank you for your question. Most probably what are you experiencing is secondary to hyperactivity of one of the expression facial muscles called levator anguli Oris superioris. Traditionally, botox used to be a good solution although it is temporary to only few months. Hi fared,Flaring of the alars while talking can be temporarily improved by botox injection or more permanently by alarplasty. Cutting the nerve is not a practical solution for this issue.
