Refilling hematoma after a facelift 11 days ago. Should I be worried? (photos)

I am very sorry about your difficult recovery.  Your situation, although it doesn't occur commonly, is something that can happen in the postoperative phase.  Close contact with your Surgeon, and possible insertion of a drain (seromacath) might be required given the recurring nature of this problem. 

If your hematoma keeps recurring, it is either because not all of the blood was originally removed, and the resulting inflammation from the blood keeps causing fluid accumulation, OR there was an injury to the parotid gland and some saliva is leaking into the cheek. You need close follow-up with your surgeon and some consideration to opening the incision to make sure all of the blood has been removed and there is no salivary gland leakage. The latter is less likely, but you do have extensive bruising, and if the fluid keeps reaccumulating, more aggressive intervention is indicated.

This is an unusual situation but it is important to keep seeing your plastic surgeon to have the fluid aspirated until it resolves. The bruising may take a little longer to resolve and it is important to avoid overactivity during this period.

You seem to have a seroma / hematoma of the face which can occasionally happen after a facelift procedure. The main concern here is skin viability and subsequent settling down of the tissues. Regular visits to your plastic surgeon with removal of fluid, when required, should resolve the problem. All The Best !

Thank you for you pictures and questions. Sorry to hear about your problems with recovery. This is a problem that needs close follow-up with your surgeon and frequent evaluation and treatment. You may need to have the fluid drained and wear compression garments to assist in preventing the problem from coming back. I would suggest you not return to work until the problem is resolved.Best of luck!

Removing the fluid should help manage the bruise.  It is improbable that the source of the hematoma is still active.  Unfortunately, facelift healing can be affected by bruising like this.

Hematoma of small amount of fluid are best aspirated, application of a pressure dressing for 48 hours after ASPIRATION IS HELPFUL, Makeup can be used so you can return to work. Good luck...

Sorry to hear of your condition.  Please follow the other consultants advice.  Hematoma is a collection of blood, outside of blood vessels under a flap, in this instance your cheek, following facelift.  Not only is it unsightly, may cause delay in your resumption of normal activities, it may result in delayed wound healing, even skin slough (death).  Hematomas can require return to the OR for reexploration or can be treated with aspiration, gentle compression and activity restriction.  Good luck.

You likely have a hematoma which is slowly liquefying, thus creating recurrent need for aspiration. This is best handle by slipping a suction canula through a small nick in the incision and removing the majority of the hematoma. Even so, the need for sequential aspiration can persist due to the irritation created by the hematoma itself. The concern is that the hematoma is going to put excess stress on the skin and effect the blood flow to the skin edges negatively impacting the healing process. You should keep on top of this seeing your surgeon frequently to make sure nothing untoward occurs.
