You are in good company with many others who had high hopes for BBL but instead received disappointing results (lumpiness, unevenness, lack of projection in the central and/or lower buttock, post-deflation sagging, etc.) as most of the fat melts away. It is not quite as much the quality of the doctor nor liposuction technique but rather the quality of the procedure that is a large part of the problem. Thus, going back for round 2 BBL most often producmmhes the same disappointing results but at another financial loss. Buttock implants indeed provide a more reliable, predictable, relatively safe, and permanent result, especially for adding projection to the buttock and often smoothing out lumpiness. Occasionally a lower buttock tuck may be advantageous for lower buttock fold asymmetries and/or sagging. Please keep in mind that an extremely few number of board-certified plastic surgeons specialize in buttock implants so be sure to consult with one that is reputable and experienced. Unfortunately you may have to travel long and far away from home to find this particular surgeon. Glad to help.

The early stages of recovery usually happen between the first and second months following surgery, while a full recovery can take up eight months to a year that is a prudent time to have a revision. Without preoperative photos to compare, I would say that you have an average good result after BBL. As you already know, take (survival) of fat grafts is unpredictalbe and it may create some volume (shape) difference from one side to another, despite the fact that the same amount of fat was placed on each side initially. It takes good 12 months for result of liposuction and fat grafting to settle and mature. If you are interested in doing more volume augmentation, I would suggest you to wait at least 12 months between surgeries to allow your fat grafts as well as donor site fat to mature and recover. Dear ncrk,One year out from your BBL you are a candidate for a revision. It would of course be dependent on additional fat being available, which is difficult to determine by photos alone. Fees for revision BBL will vary among surgeons, as the fat survival is somewhat unpredictable and the results, if unsatisfactory, may be no one's "fault." Discuss it with your surgeon, who will likely go over your before and after photos, which likely show your improvements. Good luck!
You may always have a revision BBL as long as you have good donor fat available. A physical exam would answer this question, but it appears in your photos that you still have upper back and possibly flank/lower back fat available for harvest.Dr. Taranow
To make an assessment on the outcome of the plastic surgery procedure we always need to see proper before and after pictures. Without seeing before pictures we have no idea what was actually done during the procedure since each person has a different candidacy for the procedure to start with.Individuals who have a flat spot on their buttocks should be told but this may not project sufficiently after fat transfer.Individuals vary in what is possible and what is not possible with this procedure. It isn’t necessarily the skill of the surgeon but some patients simply are not perfect candidate for the procedure.Candidacy and surgeons skill are probably about equally as important when it comes to consistent quality results.Someone who is an excellent candidate has the potential to get an excellent outcome if the procedure is done by a sufficiently skilled provider. someone who is an excellent candidate may also get a less than perfect outcome if the procedure was done with someone lacking sufficient skill and experience.On the other hand a patient who is not a great candidate for a BBL is never going to have a great result even if I’m done by the best provider in the world.If you’re not happy with your outcome then you should follow up with your provider. If that is not sufficient then I suggest you schedule in person second opinion consultations with other local board-certified plastic surgeons in your community.For a second opinion consultations patient should come repaired bringing with them all up before and after pictures and a copy of your operative report.
