Scalp advancement with/without anchoring - is there much of a difference in the outcome?

Question

I noticed some surgeons will anchor the advanced scalp during the procedure, but a few surgeons stated they don't anchor the advanced scalp, is there much of a difference in the outcome, or perhaps an alternative method that prevents tension on incision site that these other surgeons use?

Hello, hope you are well. The decision to use anchoring is decided on a case by case basis. The benefits of anchoring the scalp advancement are that it will reduce tension on the wound closure and help to prevent the scalp from migrating posteriorly while healing. Not every case requires this, but in general there are several advantages and few disadvantages. For this reason, my bias is to include fixation in the procedure.

Great question. Anchoring the scalp prevents migration upward and keeps the hairline in a stable position while it heals. There are several options for anchoring....bone tunnels made with a drill or an Endotine. The endotine will slowly dissolve over time so it will go away (and you won't be able to feel it) and should not be an issue long term. Another reason to anchor the scalp is that it takes tension off of the new incision which will help things heal better and prevent widening of the scar. The scar is typically a angled or beveled incision so as it heals the hair grows through it and the incision is less noticeable. Hope this helps!
