My eyes are pretty small and round so I would like to change their shape. I've read about a upper lateral canthoplasty but it seems like most doctors do not recommend it for a Caucasian eye and I definitely do not want to disfigure my face. I do not want any botox recommendations.
Answer: Tightening the lower eyelid Thanks for your question and posting photos. To achieve a change in shape of the eyelids, usually a canthopexy or canthoplasty is recommended. This is a procedure where the lower lid is tightened and the outer edge moved to a slightly higher position. The procedure can be done for a number of reasons (both cosmetic and reconstructive) with the goal being a tightened lower lid and an improved appearance. An in person exam would be needed to determine the amount of laxity you have in the lower lids, in addition to getting a full sense of the anatomy around the lower eyelids. Seeing a board certified or board eligible plastic surgeon will allow you to get an in depth exam and personalized treatment recommendations. Answer: Canthoplasty NOT enough for almond eyes Creating almond eyes is NOT just canthoplasty. Canthoplasty alone will distort your eye shape. To give natural, long-term almond eye results, the lower eyelids (especially outer portion) need to be raised as well. Seek an experienced surgeon.

Answer: Canthal tightening options Hi and thank you for reaching out for an expert opinion about your concern. Although an in person consultation will be necessary before a final recommendation can be made, what I see in your photos is rounding of the lateral canthus angle (the outer corner of your eyes). This is usually caused by a stretching and lengthening of the tendon in that area. So tightening of this tendon and attaching the corners close to the bony rim in that area would have the effect you are seeking. Whether a canthoplasty, canthopexy or full thickness reconstruction (tarsal strip) would be necessary, can be determined at the time of the consultation. Again, given the fact that you are young, and have beautiful eyes that have not been previously affected by trauma or surgery, my recommendation would be conservative for you. Do the minimum necessary to create the change you desire.

Answer: Canthoplasty A lateral canthoplasty can achieve what you are looking for by narrowing the lateral angle and give a more almond shape to your eye. This should be performed with an internal SOOF elevation to avoid a future descent. This surgery is performed under local anesthesia. Since you are in Baltimore, I will be glad to see you in a free consultation to explain further how it is performed and what to expect.

Answer: Canthoplasty A canthoplasty could certainly be performed to create a more desirable almond eye shape for the Caucasian eye. I would recommend seeking out a Board-certified surgeon to consult with to determine how best to achieve your goals.
