I have attached photos of my current state, original before/afters from 2015 when I had my first BA. This size is WAY too large for me now, at this point in my lift the gawking and negative connotations to having such "in your face" breasts is too much for me. I'm doing the lift to bring them back up to above the majority of my ribcage (no more sag!) and want them high & perky. Would be willing to have more spacing between if means they'd be perkier. Worried more about them being too big again.

That tear through the muscle might be somewhat difficult to repair. I would avoid trying to push the breast up and in and wearing a strap at night. A fat graft will not be helpful. Going larger may make it show more. Going smaller and wearing a strap may help.
Although you can rupture your implant at any time, without any trauma, it is unlikely that this is ruptured. I would suggest getting an ultrasound to check the implant. You also may have something going on with your neck or shoulder and need to see your medical physician.

I almost always place implants submuscular. It lowers the rate of capsular contracture significantly. In addition, it looks much more natural because the muscle provides covering over the implant so its not as round on the top. I've also noticed the implants drop less over time...
